Job Description

The SCN Program Manager will oversee the day-to-day operations of the SCN team and ensure members are screened, enrolled, and connected to services accurately and efficiently.

Responsibilities include:

• Lead, train, and support screeners and other SCN staff.
• Monitor staff productivity, quality, and follow-up.
• Ensure screenings, eligibility reviews, documentation, and service delivery are completed correctly.
• Track cases from initial screening through enrollment and service completion.
• Resolve issues with Unite Us, PHS, service providers, and internal departments.
• Maintain compliance with program requirements and billing guidelines.
• Review program data, identify workflow problems, and implement improvements.
• Assist with staff scheduling, hiring, and onboarding.
• Provide regular reports to leadership on volume, performance, and outstanding issues.
• Take ownership of expanding and improving the program.
