Job Description
The SCN Program Manager will oversee the day-to-day operations of the SCN team and ensure members are screened, enrolled, and connected to services accurately and efficiently.
Responsibilities include:
- Lead, train, and support screeners and other SCN staff.
- Monitor staff productivity, quality, and follow-up.
- Ensure screenings, eligibility reviews, documentation, and service delivery are completed correctly.
- Track cases from initial screening through enrollment and service completion.
- Resolve issues with Unite Us, PHS, service providers, and internal departments.
- Maintain compliance with program requirements and billing guidelines.
- Review program data, identify workflow problems, and implement improvements.
- Assist with staff scheduling, hiring, and onboarding.
- Provide regular reports to leadership on volume, performance, and outstanding issues.
- Take ownership of expanding and improving the program.

- Comfortable working with spreadsheets, reporting systems, and online platforms.
- Able to understand and apply detailed program requirements.
