Job Description

We are seeking a Case Manager to join our team in a hybrid role. The position offers a salary between 50000 and 60000, based on experience. You will be responsible for coordinating care for approximately 25 patients, completing state paperwork, de-escalating issues, and conducting 8 in-person home visits per month across the 5 boroughs.

Training will be conducted in-person for 4 to 6 weeks, after which the role will be remote. The schedule is Monday to Friday, with availability from 9 AM to 5 PM.
