Job Description

As a Case Manager, you will be the point person ensuring that all the moving pieces run smoothly for a caseload of approximately 25 patients and their families. You’ll provide guidance, support, and problem-solving to help them navigate challenges as they arise.


Schedule:

Monday–Friday, 9–5 availability

Crown Heights – 4–6 weeks of full-time, in-person training

After training – remote work, with required patient visits across the 5 boroughs


Position Responsibilities:

  1. Check in with patients over the phone
  2. De-escalate and problem-solve when issues arise
  3. Complete state paperwork accurately and meet deadlines
  4. Conduct 8 monthly in-person home visits across the boroughs, entering patient homes
  5. Collaborate with patients, families, and providers to ensure quality care

Requirements:

  1. BA or Associate’s degree required
  2. Strong organizational, communication, and problem-solving skills
  3. Ability to travel throughout the five boroughs
