Job Description
As a Case Manager, you will be the point person ensuring that all the moving pieces run smoothly for a caseload of approximately 25 patients and their families. You’ll provide guidance, support, and problem-solving to help them navigate challenges as they arise.
Schedule:
Monday–Friday, 9–5 availability
Crown Heights – 4–6 weeks of full-time, in-person training
After training – remote work, with required patient visits across the 5 boroughs
Position Responsibilities:
- Check in with patients over the phone
- De-escalate and problem-solve when issues arise
- Complete state paperwork accurately and meet deadlines
- Conduct 8 monthly in-person home visits across the boroughs, entering patient homes
- Collaborate with patients, families, and providers to ensure quality care
Requirements:
- BA or Associate’s degree required
- Strong organizational, communication, and problem-solving skills
- Ability to travel throughout the five boroughs
