A mental health clinic in Brooklyn, NY is seeking a Care Management Director to lead and oversee its care management program. This position is open due to an internal promotion.

The Director will supervise a team of 4 care managers, with plans to expand the department as the clinic grows. The role focuses on monitoring cases, reviewing reports, ensuring compliance, and supporting the care management team.

Responsibilities

• Supervise and support the care management team
• Review reports and documentation to ensure compliance
• Monitor cases and ensure notes and reports are completed properly
• Provide guidance on complex cases when needed
• Maintain compliance with program and documentation requirements
• Help onboard and support new care managers as the program grows
• Work closely with clinical and administrative staff

• Experience in care management or behavioral health
• Leadership or supervisory experience preferred
• Experience in clinical or remote care coordination environments
• Strong organizational and documentation review skills
• Strong communication and leadership abilities
