The role includes managing a caseload, working with families and providers, and ensuring patients receive the support they need to avoid nursing home placement.

Key Responsibilities

1. Coordinate care services for seniors living at home
2. Communicate with families and service providers
3. Arrange home care, medical services, and transportation
4. Maintain accurate documentation and reports
5. Conduct monthly check-ins
6. Perform quarterly home visits

Requirements

1. Associate’s degree
2. Strong communication skills
3. Compassionate, patient, and detail-oriented
4. Organized, reliable, and a team player
5. Ability to handle sensitive situations calmly
6. Experience is a plus but not required.

Compensation

50,000 - 60,000
