🏥 Care Meanager – Brooklyn/NYC Area

💲 $29.87–$31.93/hour | Flexible Hybrid Schedule

Advance Care Alliance NY, a leading nonprofit Care Coordination Organization, is seeking a compassionate and organized Care Manager to support individuals with intellectual and developmental disabilities. The Care Manager will work closely with members, families, providers, and interdisciplinary teams to develop and oversee personalized Life Plans, coordinate services, advocate for member needs, and ensure access to medical, behavioral health, community, and social supports.

Responsibilities include:

• Conducting assessments and reassessments

• Coordinating healthcare and support services

• Maintaining documentation and compliance standards

• Assisting with Medicaid, SNAP, Social Security, and other benefits

• Supporting transitions of care and crisis situations

• Traveling throughout the service area to meet with members and providers as needed

Qualifications:

• Bachelor’s degree + 2 years relevant experience, RN + 2 years experience, or Master’s degree + 1 year experience

• Strong communication and organizational skills

• Ability to work independently and collaboratively

• Commitment to serving individuals with I/DD and their families

This is a flexible work environment position with a blend of field work, work from home, regional travel, and office expectations.
