Overview

We're looking for an energetic, people-focused Intake Coordinator to join a growing home care organization during an exciting stage of expansion. This is a high-volume, fast-paced role for someone who enjoys helping others, thrives on the phone, and takes ownership of every patient interaction from the initial inquiry through the intake process. Success in this role requires strong communication skills, healthcare experience, attention to detail, exceptional follow-through, and the ability to manage multiple priorities without letting anything fall through the cracks. You'll play a key role in creating a positive experience for patients and their families while ensuring each case moves efficiently from initial contact through approval for services.

Responsibilities

  • Serve as the first point of contact for prospective patients and their families.
  • Manage a high volume of inbound and outbound calls.
  • Make proactive outbound calls to prospective patients and referral sources.
  • Verify patient eligibility and collect all required documentation.
  • Guide patients through the intake process from initial inquiry through approval for services.
  • Coordinate the authorization process and ensure required information is submitted accurately and on time.
  • Maintain accurate patient records and document all communication throughout the intake process.
  • Follow up with patients, families, payors, and internal team members to keep cases moving forward.
  • Work cross-functionally with the operations team to ensure a seamless transition from intake to care.
  • Take ownership of each case from initial contact through completion, ensuring nothing falls through the cracks.
  • Deliver a friendly, professional, and compassionate experience in every interaction.
  • Help build and improve intake processes as the department continues to grow.


Qualifications

  • Healthcare experience required.
  • Knowledge of MCOs, HMOs, eligibility verification, and authorizations.
  • Excellent phone presence and verbal communication skills.
  • Friendly, energetic, and people-oriented with outstanding customer service skills.
  • Highly organized with strong attention to detail.
  • Ability to manage multiple priorities in a fast-paced environment.
  • Proactive, resourceful, and solutions-oriented.
  • Strong follow-through and accountability.
  • Comfortable learning new systems and technology.
