We are a fast-growing Home Health Care company seeking an energetic, relationship-driven Community Outreach & Marketing Representative to expand our presence throughout the county.

Key Responsibilities

  • Develop and maintain relationships with social workers, case managers, hospitals, nursing facilities, home care agencies, attorneys, financial planners, independent living centers, community organizations, and healthcare providers throughout Essex County NJ.
  • Conduct regular in-person outreach visits and presentations to referral partners.
  • Represent the company at conferences, networking events, health fairs, and community programs.
  • Educate professionals about our Pooled Trust services and enrollment process.
  • Identify new referral opportunities and cultivate long-term partnerships.
  • Meet outreach and referral goals while maintaining the highest level of professionalism.
  • Collaborate closely with the intake team to ensure a seamless client experience from referral through enrollment.
  • Maintain accurate records of outreach activities and referral relationships.
  • Stay informed about Medicaid, community resources, and industry trends relevant to the populations we serve.

Qualifications

  • Excellent communication, presentation, and relationship-building skills.
  • Previous experience in healthcare marketing, community outreach, provider relations, business development, admissions, social services, Medicaid, long-term care, or a related field preferred.
  • Strong networking and public speaking abilities.
  • Self-motivated with excellent organizational and time-management skills.
  • Comfortable working independently while managing a statewide territory.
  • Proficient in Microsoft Office and CRM or database systems.
  • Valid driver's license and willingness to travel throughout New York State.
