Responsibilities:
- Provide case management services to individuals with mental health needs.
- Develop and implement individualized service plans.
- Coordinate with other service providers and community resources.
- Monitor client progress and adjust plans as necessary.
- Advocate for clients' needs and rights.
Requirements:
- Bachelor's degree in social work, psychology, or related field.
- Experience in mental health services preferred.
- Strong communication and interpersonal skills.
- Ability to work independently and as part of a team.
