NOW HIRING: Service Coordinator – Home Care

📍 Crown Heights, Brooklyn

💰 $50000–$60000 + Monthly Bonus

🕘 Monday–Friday | 9 AM–5 PM

A growing home care agency is looking for an organized, personable, and compassionate Service Coordinator to help clients access the services and resources they need.

Responsibilities

• Coordinate services and community resources for clients
• Communicate with clients, families, and service providers
• Manage and monitor an assigned caseload
• Conduct quarterly home visits to assess client needs
• Maintain accurate documentation and required reports
• Follow up to ensure services are being delivered properly

• Associate or bachelor’s degree required
• Strong communication and people skills
• Organized, responsible, and detail-oriented
• Able to manage multiple clients and priorities
• Strong follow-through and documentation skills
• Comfortable working independently and as part of a team
