Job Description

Schedule: Full Time, Mon-Fri, 9am–5pm

Pay: $25–$30/hour + commission

Location: Williamsburg, Brooklyn (in-person)

As a Case Manager, you'll manage a caseload of patients, connecting families with the right caregiver for their needs and keeping things running smoothly day to day. This role is a great fit for someone with strong communication skills who can think on their feet and stay organized in a fast-paced environment.

What you'll be doing:

1. Match caregivers with patients based on family needs
2. Re-staff cases quickly when last-minute cancellations come up
3. Coordinate scheduling and communication between patients and caregivers
