Job Description

A Crown Heights, NY Medicaid and Homecare company is hiring a service coordinator. The job includes managing a case load of patients by assisting them with finding medical providers, coordinating patient transportation, getting medical supplies, and accessing community resources. The job involves maintaining contact with the families and completing all required documentation. Once every 3 months, the candidate will be required to do a house visit (transportation is provided).

Requirements

• Must have a bachelor's degree or an associate's degree
• Compassionate with excellent communication skills
• At least 6 months of office experience in any field
