Job Description

A healthcare organization in Brooklyn is looking for licensed PAs and NPs to provide patient care both on-site and through telehealth. This role is suited to a provider who communicates warmly with patients, stays on top of documentation, and enjoys helping people access the care they need.

Responsibilities

• Assess patients and provide care within your scope of practice
• Conduct on-site and telehealth visits
• Develop care plans and coordinate follow-up
• Complete and finalize clinical documentation promptly
• Communicate clearly with patients and the care team

• Active PA or NP license
• Currently enrolled and credentialed to bill Medicaid for the services provided
• Strong clinical judgment and patient communication skills
• Comfortable with electronic health records and telehealth visits
• Reliable, organized, and attentive to documentation
