Job Description

Position Summary:

The Case Manager/Social Worker supports older adults and their families or caregivers by assessing needs, coordinating services, and managing ongoing care. This role involves regular home visits, intake screenings, referrals to resources, documentation, and care planning. The Social Worker also plays a key role in maintaining data systems, meeting grant deliverables, and assisting with the daily operations of the Older Adult Center. Strong collaboration with internal departments and external organizations is essential.


Duties & Responsibilities:

  • Conduct regular home assessments and in-home visits with clients
  • Screen all intake calls received by the Senior Division
  • Provide information and referrals to older adults and caregivers
  • Research, update, and maintain a local resource directory for client use
  • Screen clients for depression, anxiety, and substance use as needed
  • Create and regularly update individualized treatment plans for clients
  • Document all case activity within 3 days of client contact
  • Track client interactions and services using PeerPlace, AWARDS, and Google Sheets
  • Maintain and update senior visitation and food distribution lists
  • Assist with daily operations of the Older Adult Center
  • Help track and meet annual grant deliverables
  • Attend weekly review meetings, individual supervision, and case management team meetings
  • Collaborate with SBH departments and external community organizations
  • Build and maintain relationships with mental health professionals and community resource providers
  • Participate in ongoing professional training and development
  • Perform other duties and special projects as assigned


Skills & Qualifications:

  • Education:
  • Bachelor’s degree required
  • Master’s degree in Social Work (MSW), Mental Health Counseling (MHC), or related field preferred
  • Experience:
  • Experience working with the geriatric population strongly preferred
  • Familiarity with New York State professional standards required
  • Familiarity with the Orthodox Jewish population
  • Skills:
  • Strong interpersonal, communication, and organizational skills
  • Ability to work independently and collaboratively within a team
  • Proficiency in Microsoft Office, Google Workspace, and client data systems (PeerPlace, AWARDS)
