DESCRIPTION:
Care Manager – Flexible Caseload
We are seeking a dedicated HH Care managers to provide
individualized support and coordination for clients. This role
offers a meaningful opportunity to make a difference while
maintaining a flexible caseload.
Responsibilities:
• Develop and implement care plans tailored to client
needs
• Coordinate services and resources to ensure optimal
support
• Maintain accurate and timely documentation
• Communicate effectively with clients, families, and
providers
Qualifications:
• Experience in care management, social work, or a
related field preferred
• Strong organizational and communication skills
• Ability to manage multiple cases efficiently
• Spanish speaking is a plus
• Must have a BA
• Spanish speaking a plus
• Minimum 1 year of experience
Job Types:
• Full-time, Part-time
• Per Diem Structure
Schedule:
Sunday - Friday
flexible hours
Salary:
$12,000 - $48,000
annually
