We’re seeking an experienced Director of Chronic Care Management (CCM) to lead and oversee all aspects of our CCM program.

The ideal candidate has a proven track record of successfully running a company or major operation, excels at building teams and processes. Has a heavy focus on compliance and is passionate about improving patient care.

Location: Monsey/Chestnut Ridge, NY

Salary: $75,000–$150,000, depending on experience.
