Job Description

 $50,000-$65,000

Crown Heights: 4-6 weeks full time, in-person training and then remote

 9-5 availability, Mon-Fri

As a care manager you will coordinate the moving pieces so that everything runs smoothly for your 25 patients and their family members, helping them navigate challenges as they arise.

Position Overview: 

- check in with patients over the phone

- de-escalate and problem solve when patients issues arise

- complete state paperwork accurately and ensure all deadlines are met

- conduct 8 home visits monthly around 5 boroughs - MUST BE COMFORTABLE GOING INTO PATIENTS HOMES 

- ⁠ BA or Associates required!
