Position Overview
As a Case Manager, you’ll coordinate care for approximately 25 patients and their families, helping them navigate challenges and ensuring all requirements are met.
Responsibilities
- Check in with patients regularly over the phone
- De-escalate and problem-solve when issues arise
- Complete state paperwork accurately and on time
- Conduct 8 in-person home visits each month across the 5 boroughs
- Maintain up-to-date records and documentation
Requirements- BA or Associate’s degree required
- Strong organizational, communication, and problem-solving skills
- Ability to travel throughout NYC as needed
Schedule- Monday–Friday, 9–5 availability
Compensation- Salary: 50000–60000 (based on experience)
