Overview

We are seeking an exceptional Care Manager Director to lead and oversee our growing Children’s Health Home program in Bronx, NY (Hybrid). This is an opportunity to make a meaningful impact in the lives of children with complex medical, behavioral health, and social needs.

Responsibilities

• Provide overall leadership and oversight of the Children’s Health Home Care Management program.
• Supervise, mentor, and support Care Management Supervisors and Care Managers.
• Ensure compliance with all New York State Children’s Health Home requirements, standards, and organizational policies.
• Monitor program performance, quality metrics, documentation, and service delivery.
• Review caseload distribution to ensure appropriate assignment and timely engagement of children and families.
• Develop systems to improve productivity, efficiency, quality, and family satisfaction.
• Conduct regular supervision, staff evaluations, coaching, and professional development.
• Monitor outreach, enrollment, assessments, care plans, referrals, and required documentation for accuracy and timeliness.
• Ensure Care Managers maintain strong relationships with families, schools, medical providers, behavioral health providers, community agencies, and other stakeholders.
• Oversee quality assurance initiatives, audits, corrective action plans, and continuous quality improvement.
• Analyze program data and performance indicators to identify trends and opportunities for improvement.
• Assist with hiring, onboarding, training, and retaining outstanding Care Managers.
• Collaborate with executive leadership on strategic planning, growth initiatives, and program development.
• Ensure staff maintain productivity expectations while providing high-quality, family-centered care.
• Support staff in managing complex clinical and psychosocial cases.
• Promote a culture of accountability, collaboration, professionalism, and compassionate service.

Qualifications

• Required: Leadership experience within a Children’s Health Home Care Management program or closely related Care Management setting.
• Strong knowledge of New York State Children’s Health Home regulations and best practices.
• Experience supervising multidisciplinary teams.
• Excellent leadership, organizational, and problem-solving skills.
• Ability to prioritize multiple responsibilities in a fast-paced environment.
• Outstanding communication and interpersonal skills.
• Experience with quality assurance, compliance, audits, and performance improvement.
• Strong computer and documentation skills.
• Bachelor’s degree required; Master’s degree in Social Work, Counseling, Public Health, Healthcare Administration, or a related field preferred.
