Service Plan Writer – Healthcare Organization - Crown Heights

Join our growing team as a Service Plan Writer and play a key role in helping seniors access essential home care services. You’ll create customized service plans that reflect each client’s unique needs and ensure they receive the support required to remain safe and independent at home.

What You’ll Do:

• Write and update individualized service plans for seniors applying for home care based on personal and medical information.
• Maintain clear, organized documentation across multiple active cases.
• Ensure all plans align with regulatory and program guidelines by thoroughly reviewing each plan prior to submission.
• Coordinate with the assigned Service Coordinator to ensure a smooth process and accurate plan.

• Excellent writing, analytical, and computer skills.
• Strong attention to detail and able to juggle multiple deadlines.
• Strong communication and time management skills.
• Experience in healthcare, case management, or service plan writing is a plus.
