Seeking a compassionate and dedicated Case Manager to join our warm and supportive team. We are looking for individuals who lead with empathy, kindness, and genuine care for the families and communities we serve. Compensation: $28-30 per hour

Hours: Monday-Thursday 9-5PM, Fridays 9-1:00 PM

Requirements: Associate’s or Bachelor’s degree required.

Location: Crown Heights (must have reliable transportation)

Responsibilities: Conducting monthly check-ins with families; completing quarterly in-home visits; meeting with care teams bi-annually; purchasing medical supplies; connecting families with valuable community resources and support services.
