Responsibilities:
- Manage and coordinate client cases from intake through ongoing services.
- Maintain accurate documentation and case records.
- Communicate with clients, caregivers, healthcare providers, and community partners.
- Ensure compliance with company policies and applicable regulations.
- Provide excellent customer service and follow up on client needs.
Qualifications:
- Strong communication and organizational skills.
- Ability to multitask and work independently.
- Proficient with computers.
- Bilingual is a plus.
