Responsibilities:

• Manage and coordinate client cases from intake through ongoing services.
• Maintain accurate documentation and case records.
• Communicate with clients, caregivers, healthcare providers, and community partners.
• Ensure compliance with company policies and applicable regulations.
• Provide excellent customer service and follow up on client needs.

Qualifications:

• Strong communication and organizational skills.
• Ability to multitask and work independently.
• Proficient with computers.
• Bilingual is a plus.
