Now Hiring: Hybrid Case Manager

Are you an organized, compassionate professional who enjoys helping others navigate challenges? Join our team as a Hybrid Case Manager and make a meaningful impact by coordinating care and supporting patients and their families throughout their journey.

Position Overview

As a Hybrid Case Manager, you will manage a caseload of approximately 25 patients and their families, ensuring they receive the support they need while coordinating care and meeting all required state guidelines.

Key Responsibilities

1. Conduct regular check-in calls with patients
2. De-escalate situations and resolve issues as they arise
3. Complete state paperwork accurately and on time
4. Conduct 8 in-person home visits each month throughout the five boroughs of NYC
5. Maintain accurate, up-to-date records and documentation

Qualifications

1. Bachelor's or Associate's degree required
2. Strong organizational, communication, and problem-solving skills
3. Ability to travel throughout New York City's five boroughs as needed

Schedule

Monday–Friday

9:00 AM – 5:00 PM availability

Compensation

$50,000–$60,000 annually, based on experience.
