Full-Time | Client Services | Brooklyn

We are seeking a compassionate and skilled Medical Social Worker / Case Manager to join our Client Services Division. This role focuses on providing holistic, client-centered support to individuals and families navigating complex medical, emotional, and social challenges.

The ideal candidate is an experienced case manager with strong therapeutic skills, excellent organization, and a deep commitment to advocacy and comprehensive care.

Position Details

  • Title: Medical Social Worker / Case Manager
  • Status: Full-time, Non-exempt
  • Schedule: Monday–Friday, 35 hours per week
  • Compensation: $60,000–$65,000 annualized (hourly position)
  • Location: Brooklyn
  • Department: Client Services Division

About the Role

The Medical Social Worker / Case Manager develops and implements individualized, holistic care plans for clients facing medical and psychosocial challenges. This position works closely with clients, families, internal teams, and community partners to ensure coordinated and effective support.

Key Responsibilities

  • Conduct comprehensive assessments of clients’ medical, mental, emotional, social, and financial needs
  • Develop individualized treatment plans in collaboration with clients and captains
  • Coordinate services and referrals across internal divisions and external providers
  • Advocate for client needs and access to appropriate resources
  • Provide supportive counseling, reassurance, and guidance during crisis situations
  • Monitor client progress and adjust care plans as needed
  • Maintain accurate and timely documentation of all case management activities
  • Attend supervision, staff meetings, and allocation meetings
  • Collaborate closely with internal teams to ensure continuity of care

Qualifications

  • MSW or Master’s degree in a mental health–related field preferred
  • Minimum of 3 years experience in social service case management and/or mental health work preferred
  • Strong therapeutic skills, including assessment and cognitive reframing
  • Excellent organizational and written/verbal communication skills
  • Proficiency in Microsoft Office, Excel, and Google applications
  • Ability to learn and use data tracking systems
  • Familiarity with the Orthodox Jewish community preferred
  • Ability to travel to clients’ homes
