Job Description:

As a Case Manager, you will be the point person ensuring that all the moving pieces run smoothly for a caseload of approximately 25 patients and their families. You’ll provide guidance, support, and problem-solving to help them navigate challenges as they arise.

Location:

• Crown Heights – 4–6 weeks of full-time, in-person training
• After training – remote work, with required patient visits across the 5 boroughs

Schedule:

• Monday–Friday, 9–5 availability

Position Responsibilities:

• Check in with patients over the phone
• De-escalate and problem-solve when issues arise
• Complete state paperwork accurately and meet deadlines
• Conduct 8 monthly in-person home visits across the boroughs, entering patient homes
• Collaborate with patients, families, and providers to ensure quality care

Requirements:

• BA or Associate’s degree required
• Strong organizational, communication, and problem-solving skills
• Ability to travel throughout NYC

Compensation:

• $50,000–$65,000 annually
