Job Description

Join a growing home care agency as a Service Coordinator. This full-time position is Monday–Friday from 9:00 AM to 5:00 PM in Crown Heights, Brooklyn. You will coordinate care, serve as a liaison between clients, families, and providers, support access to community resources, conduct quarterly home visits, maintain documentation, and monitor service delivery.

We offer a competitive salary ranging from $55,000 to $65,000.
