Job Title: Care Manager

Location: New York State / Remote (with quarterly in-person visits)

Pay: $30+/hour (varies by experience)

A growing organization is seeking a Care Manager to support individuals by coordinating services, conducting assessments, and ensuring continuity of care. This role involves outreach, care planning, documentation, and collaboration with providers and families.

Requirements:

• Bachelor’s degree required

• Must be based in New York State

• Strong communication, organization, and follow-up skills

• Comfortable working remotely with occasional travel within NY (and limited U.S. travel if needed)
