Job Description

Salary: $55,000-$65,000

Location: Crown Heights, Brooklyn, NY

Hours: Monday-Friday, 9:00 AM-5:00 PM

We are a growing home care agency seeking a Service Coordinator to support clients in accessing essential services and community resources. This role focuses on coordinating care, monitoring client needs, and ensuring individuals receive the support required to remain safely in their homes and communities.

The ideal candidate is organized, compassionate, and able to manage multiple responsibilities while maintaining strong communication with clients, families, and providers.

Responsibilities Include:

  • Serve as a liaison between clients, families, and service providers.
  • Support clients in accessing community resources and programs.
  • Conduct quarterly home visits to assess client needs and provide support.
  • Maintain accurate documentation and prepare required reports.
  • Coordinate and monitor the delivery of services to ensure quality and satisfaction.

Qualifications:

  • Associate or bachelor’s degree required.
  • Strong interpersonal and communication skills.
  • Ability to work independently and manage a diverse caseload.
  • Organized with strong attention to detail and documentation.
