Serenity Community Care is a mission-driven organization providing high-quality, culturally responsive Health Home Care Management services across New York State. Our Lakewood-based administrative team is seeking a knowledgeable and proactive Senior Care Manager Supervisor to support our growing operations and strengthen oversight of clinical and administrative functions.
Position Summary:
The Senior Care Manager Supervisor is a leadership role combining clinical supervision with operations and quality management. This full-time, on-site position is ideal for a seasoned care management professional who thrives in a collaborative environment and can balance staff supervision with program improvement efforts.
This individual will maintain a small supervision caseload, provide oversight on case management services, HCBS referrals, lead documentation review processes, and assist in the development and refinement of workflows to ensure operational excellence and regulatory compliance across all care management divisions.
Key Responsibilities:
- Supervise a caseload of Care Managers, providing guidance on case planning, service coordination, and documentation.
- Carry a small caseload to maintain direct service experience and model best practices.
- Review and approve HCBS referrals and related documentation, ensuring timely submission and adherence to regulatory guidelines.
- Lead internal documentation audits and contribute to compliance monitoring efforts.
- Develop and improve workflows, templates, and supervision tools to optimize service delivery and ensure adherence to DOH and lead Health Home expectations.
- Collaborate closely with the QA/Compliance team to uphold audit readiness and address any performance or compliance concerns.
- Contribute to the design and implementation of training materials and onboarding processes for new staff and supervisors.
- Provide cross-division support to help manage program logistics, address operational challenges, and maintain high standards of care.
- Monitor key performance indicators (e.g., monthly contact completion, timely assessments) and help troubleshoot any barriers to quality care.
- Serve as a resource for clinical questions and escalate complex cases as needed.
Qualifications:
- Bachelor’s degree required; Master’s in Social Work, Public Health, or a related field strongly preferred.
- Minimum 2–3 years of HHCM experience, with at least 1–2 years in a supervisory or QA/compliance support role.
- In-depth knowledge of NYS Health Home Care Management and HCBS eligibility and services.
- Experience in documentation review, quality assurance, and regulatory compliance.
- Familiarity with platforms such as AWARDS/Foothold and ability to analyze data and documentation trends.
- Strong communication, leadership, and organizational skills.
- Ability to take initiative and balance competing priorities in a fast-paced environment.
