A large Boro Park, Brooklyn services company is hiring a care manager. Job involves working with adults with mental health issues or serious medical illnesses by coordinating, advocating, and linking the individuals to medical care, medication, transportation, and or therapy. Looking for someone with at least 1 year of prior experience working with adults or children as a teacher, coordinator, or anything in the human service field. Candidate must have a degree. Salary is commensurate with experience and hours.
Mon-Thurs, 9 am-3 pm, 9 am-4 pm or 9 am-5 pm, and Fri, 9 am-1/2 pm
