Responsibilities Include:

-Maintain regular communication with families

-Coordinate and manage the comprehensive care plans for patients 

-Keep detailed and accurate records of client interactions & care plans 

- Address and resolve any issues or challenges that may arise during the course of care 

-Monitor and evaluate the quality of care provided, identifying areas for improvement and implementing necessary changes to enhance service delivery
