Part time (25 hours a week) Remote, Case Management

Key responsibilities include:

• Communicating with patients and caregivers 
• Working with service providers
• Staying on top of emails and deadlines 
• Dealing with customer service and retention 
• Multitasking in an in-office environment
• Visiting patients in your caseload once every 3 months 

Requirements:

• Customer service skills
• Detail oriented
• Ability to work in a fast-paced, dynamic environment
• Excellent analytical and problem-solving skills

Location: Remote 

Salary: $35,000 - $45,000
