Care Manager Supervisor
We are seeking a dedicated and experienced Care Manager Supervisor to lead, support, and develop a team of Health Home Care Managers. In this role, you will oversee case management operations, ensure compliance with regulations, and provide guidance, training, and supervision to Care Managers serving members across New York State.
About Us
We are a small but mighty administrative team managing a growing Health Home Care Management program. While our Care Managers work in the field, our supervisors and program administrators are based in our Lakewood, NJ office, where we foster a collaborative and solutions-focused environment. We are looking for someone detail-oriented with strong leadership skills who can bring organization, problem-solving abilities, and a proactive mindset to our team.
Key Responsibilities
- Supervision & Training – Provide regular one-on-one and group supervision, ensuring Care Managers have the tools and knowledge needed to support clients effectively.
- Quality & Compliance Oversight – Monitor casework for timeliness, accuracy, and adherence to DOH, Health Home, and agency standards.
- Team Leadership & Support – Manage a team of 8+ Care Managers with mixed acuity caseloads, up to 40 cases per Care Manager.
- Documentation & Billing Review – Ensure care management activities are properly documented and meet quality and billing standards.
- Collaboration & Case Oversight – Work closely with the Director of Client Services and Quality Assurance teams to ensure program effectiveness.
- Policy Implementation – Maintain compliance with all state, Health Home, and agency regulations while supporting program development and sustainability.
Qualifications
- Bachelor’s degree required; Master’s degree in Social Work, Human Services, or Public Health preferred.
- 2–3 years of experience in care management, case management, or supervision.
- Strong leadership, communication, and organizational skills.
- Ability to handle multiple priorities and meet deadlines.
- Familiarity with case management software and documentation standards.
