Schedule: Full Time, Mon-Fri, 9am–5pm

Salary: $50,000–$60,000/year

Location: Williamsburg, Brooklyn (in-person)

What you'll be doing:

1. Figuring out why a patient's care paused or is at risk
2. Having conversations with patients & families to build trust and relationships
3. Teaming up with staffing & clinical teams to solve problems fast
4. Owning your cases from first call to full resolution
5. Following up consistently until each case is fully resolved

No healthcare experience needed - just strong communication, persistence, and a genuine desire to help people.
