Are you passionate about helping others and making a meaningful impact in their lives? We are seeking a motivated and compassionate Care Manager to join our team. In this role, you will provide outreach and enrollment services to children who meet the eligibility requirements of New York State’s Children’s Health Home program.

Key Responsibilities:

• Conduct outreach to families and children presumed eligible for the program.
• Guide and support families through the enrollment process.
• Work collaboratively to ensure comprehensive care coordination.

Qualifications:

• Bachelor’s degree or equivalent is required.
• Strong multitasking and organizational skills.
• A passion for helping others and a commitment to making a difference.
