Responsibilities:
- Assessing Client Needs: Conduct thorough assessments (CANS, comprehensive assessments, etc.) to understand each client’s physical, mental, emotional, and social health needs.
- Care Planning: Develop personalized care plans that outline the services and resources needed to address the client’s health and social care needs.
- Coordinating Services: Link clients to medical, mental health, social services, and community resources, ensuring timely and appropriate access to care.
- Monitoring Client Progress: Regularly follow up with clients to ensure they are receiving the services outlined in their care plan, and monitor their progress towards achieving health goals.
- Advocating for Clients: Act as a liaison between clients and healthcare providers, advocating for the best interests of the client in their treatment and care decisions.
- Education and Empowerment: Provide clients and their families with education on health management, available resources, and the importance of maintaining their care plan.
- Documentation: Keep detailed and accurate records of all client interactions, services provided, and progress made, in accordance with agency and regulatory standards.
- Problem-Solving and Crisis Management: Address any issues or barriers clients may face in receiving services, and provide solutions to help resolve any challenges that arise.
- Collaboration: Work with healthcare teams, service providers, and community organizations to ensure seamless care coordination and to advocate for the client’s needs.
- Confidentiality and Compliance: Ensure that all client information is handled confidentially and complies with relevant laws, such as HIPAA.
- Performance Tracking: Regularly assess and report on the outcomes of care plans, ensuring clients are receiving the services that align with their needs.
- Client Engagement: Meet with clients in person as needed to build trust, provide emotional support, and ensure they are actively involved in their care plan and decision-making process.
Requirements:
- Bachelor's Degree and 2+ years of experience working with underserved populations or a Master's Degree in Social Work/other Human Services field
- Excellent written and verbal communication.
- Technology-proficient. (Use of Microsoft Office & other documentation programs required).
- NYS Health Home care management/HCBS/NY Waiver program experience preferred.
- Yiddish/Spanish bilingual is a plus.
- Must hold an active NYS Driver's License or have other means of reliable transportation
