Job Description

As a case manager you will guide prospective patients through the enrollment and onboarding processes for ABA services and support their ongoing needs with the home care provider(the BCBA and BT). Your role provides prospective and active participant and family support with frequent communication and follow up, verification of eligibility for assistive programs and maintenance of accurate and comprehensive documentation. As well as, pairing the correct BCBA with prospective patient that fits their needs. By ensuring timely interventions, you will contribute to improved patient outcomes.
