The ideal candidate will be proactive in implementing case management policies, adept at communicating with client families and stakeholders, and dedicated to fostering wellness for our clients and their families. Our Case Managers play a crucial role in ensuring that the cases under their purview are consistently staffed.

Responsibilities:

• Establish and maintain regular, supportive communication with client families, including monthly check-in calls
• Actively troubleshoot any issues that may arise with services provided
• Ensure timely receipt of patient progress reports and assist with insurance reauthorization processes
• Maintain confidentiality of client records
• Manage staffing needs within the region, collaborating closely with our recruiters to ensure appropriate case staffing
• Conduct candidate interviews and facilitate timely case staffing
• Develop work schedules, allocating employees to shifts and positions as needed
• Document all communication with client families and service providers
