Job Posting: Field RN Supervisor (Brooklyn)

Schedule: Full-time (36 hours/week)

Reports to: Director of Patient Services

Location: Field-based, Brooklyn /NYC region

About the Role

We are seeking an experienced and dedicated Field RN Supervisor to oversee skilled nursing services and support high-quality patient care across our Downstate region. This role provides hands-on clinical leadership, supervises nursing staff, ensures regulatory compliance, and contributes to staff education, audits, and workflow development.

Key Responsibilities

• Train, mentor, and onboard new RN staff
• Oversee field nursing services and support caregivers, nurses, and therapists
• Conduct internal audits to ensure DOH, accreditation, and agency compliance
• Assign cases and manage RN workloads
• Collaborate with HR and Coordination teams on staff training
• Perform RN performance evaluations
• Provide patient and family education
• Maintain accurate and timely clinical documentation
• Lead emergency preparedness drills and policy-related training
• Manage incident reports, DOH complaints, and follow-up
• Update policies and maintain agency policy manuals
• Support the DPS during audits and workflow development

Qualifications

• Current NYS RN license
• Graduate of an accredited nursing program (Diploma, BSN, or MSN)
• Experience in home care preferred
• 2+ years of supervisory/management experience
• Strong clinical assessment and decision-making abilities
• Excellent communication, organization, and documentation skills
• Knowledge of DOH regulations, accreditation standards, and home care best practices
