Job Description

Job Summary:

We are seeking an experienced and dedicated HCBS Intake Director to oversee the intake process for Home and Community-Based Services (HCBS). This leadership role involves managing and supervising HCBS Coordinators and ensuring the efficient processing of referrals from start to finish. The HCBS Intake Director will be responsible for streamlining referral workflows, ensuring compliance with regulatory standards, and providing support to both staff and clients throughout the intake and service initiation process.

Key Responsibilities:

  • Supervise HCBS Coordinators: Lead, manage, and provide ongoing guidance to a team of HCBS Coordinators. Ensure the team delivers high-quality intake services, meets performance expectations, and adheres to organizational policies and procedures.
  • Referral Management: Oversee the referral intake process from initial contact to service initiation. Ensure that all referrals are processed accurately, in a timely manner, and in compliance with applicable regulations.
  • Client Needs Assessment: Work closely with HCBS Coordinators to assess and verify client needs, ensuring appropriate services are recommended. Ensure that comprehensive documentation is maintained for each client.
  • Collaboration with External Stakeholders: Build and maintain relationships with healthcare providers, community organizations, and other stakeholders to ensure timely and accurate referrals.
  • Quality Assurance: Monitor and evaluate intake processes and outcomes. Implement quality improvement initiatives to streamline operations, reduce errors, and enhance customer satisfaction.
  • Compliance and Documentation: Ensure all documentation related to client referrals, assessments, and service plans is complete and compliant with federal, state, and organizational guidelines. Conduct regular audits to ensure compliance.
  • Training and Development: Provide training, coaching, and professional development opportunities for HCBS Coordinators to enhance their skills and knowledge of best practices, regulatory requirements, and organizational policies.
  • Problem-Solving and Escalation: Act as the point of escalation for complex or challenging cases. Provide guidance and solutions to staff, ensuring client issues are resolved efficiently.
  • Data Management and Reporting: Monitor and report on referral trends, service gaps, and program performance. Collaborate with leadership to develop strategies to address areas for improvement.
