SOCIAL WORKER POSITION The social worker will collaborate with the administrative team, Case Management, Physicians, and Nursing staff to meet the needs of the patients and their families. The Social Worker is responsible for Family meetings, Assessments, Discharge planning, and emotional support for patients and families.
