Hybrid Job Opportunity: Service Coordinator
Description:
Make a real impact by helping patients with Alzheimers/Dementia stay independent at home. As a Service Coordinator, you’ll manage a caseload of 25 patients, ensuring they receive the care and resources they need. This is a hands-on, fast-paced role with a mix of patient interaction and administrative work.
Salary: $50-60K
Location: Hybrid (Must be in NYC for in-person visits + 2 weeks in person training)
What You’ll Do:
- Communicate with patients & family members
- Coordinate transportation & supplies
- Keep patient records up to date & meet deadlines to submit paperwork
- Troubleshoot issues with patient and family to ensure smooth service
- Visit patients in their homes through the 5 boroughs, to ensure services are running smoothly
What We’re Looking For:
- B.A. REQUIRED
- Strong communication & customer service skills
- Detail-oriented & organized
Able to multitask in a fast-paced environment
- Available 9-5 for patient calls
