Hybrid Case Coordinator

Oversee 25 patients receiving full-time care — from the comfort of your home 🤌

Salary: $50,000 - $60,000

Location: Crown Heights (Hybrid)

Schedule: 9 AM - 5 PM, Mon-Fri

Overview of Responsibilities:

☎️ Check in with patients over the phone

🤝 De-escalate when patients are dissatisfied

📝 Complete paperwork by deadlines

🛌 Conduct 8 home visits monthly around NY

🎓 BA or Associate's degree required!
