Job Description

Responsibilities:

  • Oversee 4-5 service coordinators, ensuring they are effectively managing their caseloads.
  • Maintain a small personal caseload, providing case management services to assigned patients.
  • Lead by example, promoting excellent communication and customer service.
  • Support and mentor service coordinators, offering guidance in resolving patient issues and improving performance.
  • Ensure compliance with paperwork and documentation deadlines.

Requirements:

  • Strong leadership skills, with the ability to motivate and guide a team.
  • Excellent customer service and communication abilities.
  • Initiative and problem-solving mindset, with a focus on patient retention and satisfaction.
  • Familiarity with case management processes and service coordination.
  • Proficiency in software tools and ability to learn new systems (Bolt CRM, HHAX, Smartsheets).

Salary: $60,000-$70,000 based on experience
