Job Description
Responsibilities:
- Oversee 4-5 service coordinators, ensuring they are effectively managing their caseloads.
- Maintain a small personal caseload, providing case management services to assigned patients.
- Lead by example, promoting excellent communication and customer service.
- Support and mentor service coordinators, offering guidance in resolving patient issues and improving performance.
- Ensure compliance with paperwork and documentation deadlines.
Requirements:
- Strong leadership skills, with the ability to motivate and guide a team.
- Excellent customer service and communication abilities.
- Initiative and problem-solving mindset, with a focus on patient retention and satisfaction.
- Familiarity with case management processes and service coordination.
- Proficiency in software tools and ability to learn new systems (Bolt CRM, HHAX, Smartsheets).

Location: Williamsburg, full-time, in-office with occasional site visits
Salary: $60,000-$70,000 based on experience
