LaSante Health Center is seeking a Medicaid Services Navigator to join our team.

The ideal candidate will help Medicaid recipients access essential health and social services, focusing on the 1115 Medicaid Waiver.

The Navigator will assist patients in navigating care management, housing, food security, transportation, and other benefits to enhance their well-being.

Responsibilities:

• Screen patients to identify social needs (housing, food, transportation).
• Educate patients on Medicaid services
• Guide patients in understanding Medicaid rights, responsibilities, and options.
• Develop and coordinate personalized service plans.
• Connect patients with housing, food, and transportation resources.
• Work closely with healthcare providers and community resources for comprehensive care.
• Assist patients with applications for housing, SNAP, and meal delivery.
• Arrange transportation for medical appointments.
• Ensure access to Medicaid-covered services (preventive care, mental health, etc.).
• Advocate for patients and assist in overcoming barriers to care.
• Maintain accurate records and meet compliance requirements.
• Stay current on Medicaid policy changes and participate in quality improvement initiatives.

Qualifications:

• Prior experience in a healthcare setting.
• Strong communication skills.
• Ability to commute to Boro Park.

Salary: Starts at $25 an hour
