Job Description

We are looking for a dynamic leader to oversee our NY Health Home Care Management program. If you have strong leadership skills, excellent communication and presentation abilities, and experience in health home care management, this role is for you.

Responsibilities

• Lead and grow the Health Home Care Management team
• Develop strategies to improve care coordination and patient engagement
• Ensure compliance with Medicaid, DOH, and OMH regulations
• Motivate and mentor staff while driving operational excellence
• Promote and pitch the program to referral sources and community partners

Requirements

• 3-5 years of experience in health home and/or case/care management
• Leadership and people management experience
• Strong presentation, program sales, and motivational skills
• Bachelor’s Degree (minimum education requirement)

Salary & Benefits

• $85K - $100K (commensurate with experience)
