Job Description

We are hiring a full-time Licensed Practical Nurse to provide 7-8 patient visits per day in their homes. This is a paid per visit role, with an estimated annual pay of $77,000-$95,000 depending on experience.

Responsibilities

The Licensed Practical Nurse (LPN) in Home Health provides quality care and technical skilled care to patients in their homes. Responsibilities include:

• Delivering services within the scope of practice as defined by state nursing laws.
• Following the plan of care in coordination with the healthcare team (may not alter the plan of care).
• Instructing the patient’s family on care and maintaining a healthy environment.
• Observing, recording, and reporting patient responses and changes in condition to the supervising nurse or physician.

Education and Experience

License Requirements:

• Current LPN licensure in the state of practice.
• Current Driver’s License, vehicle insurance, and access to a dependable vehicle or public transportation.
• CPR Certification is required.
